BCBA® Fieldwork Tracker – Supervision Hours Log – MULTIPLE Supervisors

If you are in the process of working toward your BCBA certification, then this product is for you! This is a digital BCBA® fieldwork tracker in which you can input all of your experience and supervision hours (aligned with 5th edition task list requirements). This tracker is designed for individuals accruing supervision experience from multiple supervising agencies.

Don’t stress over manually calculating monthly hours, percentages of restricted/unrestricted hours, or percentage of supervised hours- let this tracker do the work for you!

Enter all required information into this Google Sheet™, including:

• Date
• Start/End Time
• Fieldwork Type
• Setting & Supervisor Name
• Activity Category (Restricted or Unrestricted)

For Supervised Hours, there are additional columns for:

• Format of supervision (in-person vs online)
• Supervision type (individual vs group)
• Whether or not the supervisor observed the trainee with clients
• Summary of supervision activity

There is a SAMPLE tab included in this BCBA® fieldwork tracker to provide you with a snapshot of what the tracker looks like with information filled in. Keep the BLANK tab blank so that you always have a blank master copy to work with. Duplicate the BLANK tab for each new month to enter in your own fieldwork experience information!

There is also a tab labeled “CUMULATIVE HOURS” to keep a record of how many TOTAL hours you’ve accrued and how many you have left to go! Simply enter in your total monthly hours at the end of each month into the Cumulative tab and the numbers will update for you in the table on the right.
