IEP Parent Survey Form for Parents or Guardian Input – Special Education IEP Prep

This IEP parent survey can be used to gain parent or guardian input into the IEP preparation process! Working together with a family on their child’s special education services and needs will foster a collaborative relationship between home and school

This 5 page survey contains:

• a brief introductory letter as to purpose of survey
• space for parents/guardians to list their child’s strengths and weaknesses by domain area
• space for parents/guardians to comment on skills they would like to see their child work on or improve upon in school

This survey was geared toward functional life skills classrooms including sections for parents or guardians to describe their child’s needs in self-help, daily living, and pre-vocational skills, as well as academic needs. The survey can be modified to suit your student’s individual needs.

There is also a note which explains to families that their requests will be considered but may not appear in a formal IEP (if it is more appropriate for them to be targeted in other ways).
